There is a second layer that catches people out. The taluka itself was renamed. Tiswadi was formerly known as Ilhas, and older land and registry papers still carry that name. Between 1961 and 1987 This Was a Union Territory Capital

Panaji has been a capital continuously since 1843, but not always of the same thing. It was the seat of Portuguese India until 1961, then the capital of the Union Territory of Goa, Daman and Diu, and only from 30 May 1987 the capital of the state of Goa. North Goa district came into being at the same time, so documents older than that do not name a district at all.

For a name change file that matters more than it sounds. A service record, a school certificate or a property paper from those years names a jurisdiction that no longer exists in that form, and no present day district. None of it is an error and none of it needs correcting. The affidavit simply has to set out the chain, naming what the document says, when it was issued and what your address is called today.

Send the oldest paper in the house: even a certificate naming Nova Goa or Goa, Daman and Diu is valuable, because that is where the chain of your name begins. Portuguese period records are welcome and we will tell you how to place them.

Not Every Case in Panaji Needs a Gazette

Many agents sell a Gazette for every case. That is not honest. A small spelling mistake is usually fixed with a notarised affidavit and your existing proof at each authority. A birth certificate error goes through the Chief Registrar at the Directorate of Statistics and then the Civil Registrar for your taluka, not through a Gazette at all.

A marriage in Goa has to be registered with the Civil Registrar to be legally recognised, so if your change follows a marriage, that registration is the base document and comes first. Caste, income and residence certificates are corrected by the authority that issued them, and property entries through the registration and revenue side. A Gazette is for a full legal name change, and for cases where the same difference repeats across several records.
