The Official Gazette of the Government of Goa is published by the Government Printing Press at Panaji, and it is issued in numbered series. Name change notices are published in Series III, alongside affidavits, notarial deeds and similar public declarations. If you have seen a Goa Gazette page with entries reading that one name and another are one and the same person, that is Series III.
Knowing the series matters, because a file addressed to the wrong series comes back. The declaration itself follows a settled pattern. It names the old name, the new name, the documents affected such as the Aadhaar, PAN, passport, driving licence and bank passbook, and it states plainly that both names belong to the same person. We draft it in that form and file it correctly the first time.

Marriage Here Runs on the Civil Registrar, Not a Certificate Alone
Goa is the one state in India that still runs on its own civil code, descended from the Portuguese Civil Code of 1867 and never replaced after 1961. One consequence matters enormously for name changes. A marriage in Goa has to be registered with the Civil Registrar, and without that registration it is not legally recognised, whichever community you belong to. The registration entry, not a ceremony record, is the document every office will ask for.
So if you are changing your name after marriage in Goa, the order is different from the rest of the country. The marriage registration comes first and becomes the supporting proof. The affidavit and the Gazette then rest on it, and after publication the Aadhaar, PAN, passport and bank records are updated. Trying it the other way round is the most common reason a Goan file stalls.
A Goa Birth Certificate Correction Takes Two Steps, Not One
In most of India a name error on a birth certificate is corrected at the municipal body that registered it. Goa works differently, and this is where people waste the most time. You first apply to the Chief Registrar of Births and Deaths at Panaji, under the Directorate of Statistics, with the documents proving the error. Only after that authorisation comes through do you approach the Civil Registrar for your taluka to have the entry corrected.
A Gazette notification does not do this and is not required for it. Anyone who sells you a Gazette to fix a birth certificate is charging for something that will not solve the problem. What a Gazette does is settle your identity once, so that every record issued afterwards carries the corrected name.
Your Old Papers May Say Goa, Daman and Diu
Goa was liberated on 19 December 1961 and became part of the Union Territory of Goa, Daman and Diu. It stayed that way for more than twenty five years, and it was only on 30 May 1987 that Goa became a state in its own right, with Daman and Diu going their separate way. Until then Goa was administered as a single district, and North Goa and South Goa came into being with statehood.
So a school certificate, a service record or a land paper from before 1987 can name a territory that no longer exists in that form, and will not name your present district at all. None of it is an error. The affidavit has to set out that chain in plain words, naming the old document, what the territory was called then, and your district and taluka today. Standard drafts leave this out, and that is when a Goan file comes back for clarification.

Not Every Case in Goa Needs a Gazette
Many agents sell a Gazette for every case. That is not honest. A small spelling mistake is usually fixed with a notarised affidavit and your existing proof at each authority. A birth certificate error goes through the Chief Registrar at Panaji and then your Civil Registrar, not through a Gazette at all.
Caste, income and residence certificates are corrected by the issuing authority, and property entries through the registration and revenue side, which matters here because so many holdings carry old descriptions. A Gazette is for a full legal name change, after marriage, after divorce, for adding or dropping a surname, or by personal choice, and for any case where the same difference repeats across several records.
