Quick Answer

To change your name legally in Quepem, you follow three steps. A notarised affidavit on stamp paper, a public notice in the newspaper, and then a Gazette notification. The Goa Official Gazette is printed at Panaji and name change notices appear in Series III. Since December 2025 Quepem is the headquarters of the new Kushavati district, and the affidavit should say so.

The Change That Affects Every File Here

This Town Became a District Headquarters on 31 December 2025

At the end of December 2025 Goa was reconstituted into three districts. A new district named Kushavati was carved out of South Goa, taking in the talukas of Quepem, Sanguem, Dharbandora and Canacona, and Quepem town was designated its headquarters. For a taluka town that is a considerable change. It also means that every document you hold from before that date names South Goa, while anything issued since names Kushavati.

Both versions are correct for their time and neither needs correcting on its own. But a name change file is checked on the name and the address together, so a verifier comparing an older certificate with a recent Aadhaar sees two districts. The affidavit has to state plainly that the town is the same and that the district was reorganised in December 2025. That single line is what most drafts are still missing, because the change is only months old.

Expect to see both for a while: the new administration is being set up in stages and the South Goa office has been handling the work in the meantime, so some listings still show South Goa. That is normal and does not affect your file when the wording is set out properly.
Central Gazette

The Central Gazette Process for a Name Change in Quepem

This is the route accepted everywhere in India and abroad. Here is exactly what happens at each step.

Prepare and Notarise the Affidavit

A sworn declaration on non-judicial stamp paper, signed with witnesses and attested by a Notary Public or First Class Magistrate in Quepem.

Publish the Newspaper Notice

A public notice carrying exactly the same wording as your affidavit, so the change is on public record.

File With the Department of Publication

The complete file goes to the Controller of Publications, Civil Lines, Delhi, and your notice appears in Part IV of the Gazette of India.

Step 1 in Detail

What Your Name Change Affidavit Must Contain

Your old name exactly as printed on your existing documents
Your new name, spelt exactly as you want it recorded forever
Your house number, ward, village and taluka, exactly as on your proof
The district as your identity proof shows it, with the older one noted
A declaration that both names belong to the same person
Your signature, witness signatures, and the notary stamp with date
The district line needs a sentence of its own: where your older paper names South Goa and your identity proof names Kushavati, the affidavit should record both with the year of the change, rather than quietly using one and hoping nobody compares.
Step 2 in Detail

The Newspaper Notice Rules

The notice must repeat the same old name, new name and address as the affidavit
It must mention the date on which the affidavit was made
For the Central Gazette, one Hindi and one English daily are required
The advertisement usually appears in the classified section
Keep the original dated cuttings showing the paper name
Even one letter spelt differently can get your whole file returned
Cost of this step: budget dailies cost about Rs 1,000 to Rs 1,500, while premium national papers can cost Rs 2,500 to Rs 5,000. We place the notices and collect the cuttings for your file.
Step 3 in Detail

What Goes Into Your Gazette File

The signed Gazette application form in the prescribed proforma
The original notarised affidavit
Original cuttings of the newspaper advertisements
A request letter addressed to the publishing authority
Identity proof, two photographs and the soft copy of the notice
The fee receipt, paid in the applicant's own name
Government fee and time: Rs 1,100 for an adult and Rs 1,700 for a minor for the Central Gazette. Publication usually takes 4 to 6 weeks after a complete file reaches the department, and the e-Gazette copy carries the same legal weight as the printed one.
Where the Name Comes From

The District Is Named After the River That Runs Past This Town

Kushavati is not a new word here. It is the river that flows through this taluka, past the Palacio do Deao which stands on its banks, and the town's own older name is said to have been Kushvan. So the town, the river and the district all share a root. That is worth knowing when you look at old papers, because a place name recorded generations ago may look unfamiliar and still refer to exactly where you live.

In practice we treat every version as evidence rather than as an error. An old land record, a temple paper or a school certificate carrying an older form of the village or town name is not something to hide from the file. It goes into the affidavit, named and dated, alongside your present address, so the chain reads as one place and one person from the earliest paper to the newest.

Send the oldest paper in the house: we read it before advising. An unfamiliar place name on an old document is usually the most useful thing a family brings us, not the biggest problem.
Compare the Two Routes

Goa Gazette or Central Gazette for Quepem?

Both are valid. Here is the honest difference, so you pick the right one the first time.

Where it appearsSeries III of the Goa Official GazettePart IV of the Gazette of India
Where it is acceptedMainly within GoaEvery state, and by all Government and private bodies in India

Our honest advice for Quepem: for a state department record the Goa route is fine. Where work has taken people outside Goa, or a passport is involved, the Central Gazette settles the question once and carries the district wording cleanly. The state level detail is on our Goa name change guide, and the full comparison is in our name change agent in India guide.
For Older Documents

Records Here Begin Later Than in Coastal Goa

Quepem has a known founding date. The town was established in 1787 by Deao Jose Paulo de Almeida, five years after this sub-district came to the Portuguese in 1782 through an agreement with the ruler of Sunda. Before that it was scattered settlement in heavy forest. The coastal talukas of Tiswadi, Bardez and Salcete had been under Portuguese administration since the early fifteen hundreds, which is a gap of more than two and a half centuries in record keeping.

For a name change file that has a practical effect. Families here often cannot produce a written record going back as far as a coastal family can, and the earliest paper may be a land entry or a church or temple record rather than a certificate. That is not a weakness in your case. What matters is that whatever the oldest document is, the affidavit names it, dates it and connects it to your present identity in an unbroken line.

If your oldest paper is not a certificate: that is common in this belt and it is still usable. Send it and we will tell you how it should be described in the affidavit.
Save Your Money

Not Every Case in Quepem Needs a Gazette

Many agents sell a Gazette for every case. That is not honest. A small spelling mistake is usually fixed with a notarised affidavit and your existing proof at each authority. A birth entry goes through the Chief Registrar of Births and Deaths at Panaji and then the Civil Registrar for Quepem, not through a Gazette at all.

A marriage in Goa is not legally recognised unless registered with the Civil Registrar, so if your change follows a marriage, that registration comes first. Land and revenue entries go through the talathi and the registration side. A Gazette is for a full legal name change, and for cases where the same difference repeats across several records.

Name Change in Quepem, Frequently Asked Questions

Clear answers to the questions we hear most often.

How do I change my name legally in Quepem?
In three steps. A notarised affidavit on stamp paper, a public notice in the newspaper, and a Gazette notification. We handle all three for you, fully online.
Which district should I write, South Goa or Kushavati?
Use whichever your identity proof shows. Kushavati district was created on 31 December 2025 out of South Goa with Quepem as its headquarters, so newer documents name Kushavati and older ones name South Goa.
My old certificate says South Goa, does it need correcting?
No. It was correct when it was issued. The affidavit simply has to state that the town is the same and the district was reorganised in December 2025.
Which talukas are part of the new district?
Quepem, Sanguem, Dharbandora and Canacona, all carved out of South Goa, with the headquarters here at Quepem.
Some listings still show South Goa for Quepem, why?
Because the change is only months old and the administration is being set up in stages, with the South Goa office handling work in the meantime. Seeing both is normal right now.
My old paper uses an unfamiliar village or town name, what do I do?
Send it. Older forms of place names are common here and they are evidence, not errors. The affidavit names the document, dates it and connects it to your present address.
Which Gazette series does a name change go into?
Series III of the Official Gazette of the Government of Goa, printed at Panaji. The central equivalent is Part IV of the Gazette of India.
How do I correct a name in a birth certificate here?
In two steps. Apply to the Chief Registrar of Births and Deaths at Panaji with proof of the error, and then approach the Civil Registrar for Quepem. A Gazette is not required for it.
How much does a name change cost in Quepem?
The Central Gazette Government fee is Rs 1,100 for an adult and Rs 1,700 for a minor. The Goa Gazette fee is separate, and newspaper and affidavit charges are extra. Our complete service starts at Rs 3,999.
How long does the whole process take?
The Central Gazette usually takes about 4 to 6 weeks after a complete file is submitted. The affidavit is ready in a day or two and the newspaper notice within a week.
