This is a harder problem than most renamings and it is worth understanding why. When Gurgaon became Gurugram, anyone can see the two are the same word. Abdullapur and Yamunanagar share no letters at all. A verifier in another state reading Abdullapur on a school record and Yamunanagar on an Aadhaar has no reason to think they are the same place, and no way to work it out. It looks like two addresses, not one. So the affidavit cannot leave it implied. It has to name the document, its year, the place exactly as printed, and state plainly that the settlement now known as Yamunanagar was formerly Abdullapur.
