It opens by calling the district Shivamoga, says in the next line that a major part of Shimoga district lies in the Malnad region, calls the administrative centre Shivamoga city, lists Shivamoga among the seven taluks, notes that Shimoga was previously known as Mandli, and then explains how the name Shivamogga evolved. Three spellings, one page, one paragraph, on a domain that is itself spelt shivamogga.

If you believe that, your name change stops before it starts, because you now have to find two Group A or Group B Government officers who are willing to sign a personal document for a stranger.
An affidavit is sworn before a Notary Public or a First Class Magistrate and signed by two witnesses. Ordinary adults. A neighbour, a colleague, a relative. Nobody has to hold gazetted rank. This is the same confusion we see everywhere between a gazetted officer, who is a Government official whose appointment was announced in a gazette and who may attest copies, and the Gazette itself, which is a publication. The two words look alike and mean entirely different things.

On 1 November 2006 the Government of Karnataka announced that Shimoga would be renamed Shivamogga, along with nine other cities in the state. The Central Government approved the request, by then covering twelve cities, in October 2014, and the city was renamed on 1 November 2014. Eight years between the announcement and the day it took effect, and the list itself grew from ten cities to twelve along the way.
If your own name change feels slow, hold that thought. A state government announced a renaming, meant it, and still needed eight years and a central approval before the new name was real on paper. Nothing about the process punishes you for the wait. It simply means the announcement and the effective date are two different dates, and only one of them belongs in a document.
Five Explanations of the Name, and the Government Picks None
The district administration's own page sets out the legends without choosing between them. One group connects the name to Shiva, giving Shiva-Mukha, the face of Shiva, Shivana-Moogu, the nose of Shiva, and Shivana-Mogge, the flowers offered to Shiva. Another legend derives it from Sihi-Moge, meaning sweet pot, from the account of the sage Durvasa's ashram, where herbs boiled in an earthen pot were tasted by cowherds who exclaimed at the sweetness. Published accounts add that Shivamogga was previously known as Mandli, which is a different name altogether.
Five explanations, and the official page presents them side by side as legends rather than declaring one correct. That is honest, and it is also the point. Even the name of a city that has been continuously inhabited for centuries is not a settled single fact with one derivation. It is several accounts that survived, each true to somebody.
Here the State Changed Around You, Not the Other Way Round
Most of our Karnataka pages explain a district that arrived from somewhere else. Belagavi came from Bombay State, Kalaburagi from Hyderabad State, Ballari from Madras. This district is the other kind. The Keladi Nayakas ruled here as an independent kingdom for about two centuries after the fall of Vijayanagara, and in 1763 the area came into the Kingdom of Mysore and stayed there through to independence. It did not transfer in during the reorganisation. Mysore State simply became Karnataka around it in 1973.
That does not mean nothing moved. In 1997 Channagiri and Honnali left this district for the newly formed Davanagere district, so papers from those two taluks issued before 1997 correctly name Shimoga district. And the city and district spelling changed in 2014. So the chain here is shorter than in Ballari or Kalaburagi, but it is not empty, and the pieces of it still have to be named correctly for their year.
Not Every Case in Shivamogga Needs a Gazette
A name error in a birth certificate is corrected by the Registrar of Births and Deaths at the body that registered the birth, under section 15 of the Registration of Births and Deaths Act, 1969. No Gazette and no newspaper advertisement are involved. An SSLC or PUC marks card is corrected by the board that issued it. Caste, income and residence certificates are corrected by the issuing authority. And if your degree prints Shimoga while your Aadhaar prints Shivamogga, that is not a case at all, because both are correct for their year.
